CRNBC (now known as BCCNP) received information in January 2016, indicating that Kuldip Takhar may be involved in unauthorized practice.

Investigation indicated Ms. Takhar was practising outside the scope of nursing in the time period of April 2014 to April 2016, by performing Botox and dermal filler injections on clients in a clinic environment but without the necessary supervision of a physician, without appropriate physician consultation of the client, and without a client specific written order.  She failed to follow established policy and procedure with respect to the administration of Schedule I and Schedule II medications.  Her documentation of the Botox and dermal filler injections was not adequate.  In sum, she failed to adhere to the following CRNBC Practice Standards: Medication Administration, Documentation, and Acting with an Order.

Ms. Takhar cooperated with the investigation.  She agreed to a condition restricting her from administering Botox or dermal filler injections until she has completed an accredited training course on this practice and has provided CRNBC (now known as BCCNP) with proof of successful completion of such training, education, and a regulatory practice consultation on practising within scope (including how to assess potential employment environments for suitability and appropriate practices prior to accepting employment).

The Inquiry Committee is satisfied that these terms will protect the public.
