Consent agreement

On December 24, 2019 a panel of the Inquiry Committee approved a Consent Agreement between BCCNP and Mark Minuhin of Burnaby, B.C. to remediate practice issues related to the following: multiple medication errors; discrepancies and omissions in documentation; inaccurate and incomplete assessments of patients; failure to provide timely interventions; failure to follow medication administration standards; failing to adhere to established policies and procedures; and, he was dishonest with a patient's family member, colleagues and management, as well as, falsifying documentation to cover up an error.

The Registrant has voluntarily agreed to terms equivalent to a limit and condition on practice, including: a requirement to work with supervision for a period of 6 months (including random chart audits with a supervisor), and a 6-month limit precluding practice as a sole RN or as the in-charge nurse or in a supervisory capacity of any kind.  He has agreed to additional terms including disclosure to employers and remedial education. 

The Inquiry Committee is satisfied that the terms will protect the public.
