Important information for MAiD providers

Please double-check MAiD forms for completeness before submitting

The Medical Assistance in Dying (MAiD) Oversight Unit at the BC Ministry of Health has identified a few key areas that are commonly being missed by providers completing MAiD forms 1633 Assessment Record (Assessor), and 1634 Assessment Record (Prescriber) forms.

To help reduce the amount of time you have to spend following up with the MAiD Oversight Unit, please carefully review your forms before you submit them. Make sure all the sections have been completed on each page of the forms, and that you have included all the pages included in each form.

Here a few specific sections to double check:

Information regarding the patient's request

This section is located on page 1 of both the 1633 and 1634 Assessment Records. Please be sure to indicate:

  • the date you received the patient's written request
  • who you received it from, and
  • whether the patient had previously consulted you for a health reason, other than seeking MAiD

 

Practitioner signature

This section is located on page 3 of the 1633 Assessment Record and page 4 of the 1634 Assessment Record. Please ensure you:

  • sign the form, once it's complete
  • provide the date and time you signed it

 

Patient's access to palliative care

This section is located on page 3 of the 1634 Assessment Record. Please note:

  • if you indicate 'yes' the patient has received palliative care, you must also indicate the length of time they received the care
  • if you indicate 'no' the patient has not received palliative care, you must also indicate whether you know or believe they had access to palliative care

 

Submitting MAiD forms by email

Beginning Aug. 12, 2019, the Ministry of Health's MAiD Oversight Unit will begin accepting MAiD forms by email, in addition to the current fax option.

NPs who wish to submit their paperwork by email may want to consult their employer and the Privacy and Confidentiality Practice Standard for guidance on secure and confidential transmission of personal information, and to ensure they're meeting their obligations under BC's Freedom of Information and Protection of Privacy Act and the Personal Information Protection Act.

The email address to submit forms is hlth.maidoversight@gov.bc.ca. Practitioners are also still welcome to submit forms by fax to 1-778-698-4678.
