The Medical Assistance in Dying (MAiD) Oversight Unit at the BC Ministry of Health has identified a few key areas that are commonly being missed by providers completing MAiD forms 1633 Assessment Record (Assessor), and 1634 Assessment Record (Prescriber) forms.
To help reduce the amount of time you have to spend following up with the MAiD Oversight Unit, please carefully review your forms before you submit them. Make sure all the sections have been completed on each page of the forms, and that you have included all the pages included in each form.

Beginning Aug. 12, 2019, the Ministry of Health's MAiD Oversight Unit will begin accepting MAiD forms by email, in addition to the current fax option.
NPs who wish to submit their paperwork by email may want to consult their employer and the
Privacy and Confidentiality Practice Standard for guidance on secure and confidential transmission of personal information, and to ensure they're meeting their obligations under BC's
Freedom of Information and Protection of Privacy Act and the
Personal Information Protection Act.
